Job Description:
A company is looking for a Care Manager (RN) to develop and
facilitate complex care management activities for members with
physical needs.
Key Responsibilities
Evaluate member needs and barriers to care, recommending plans for
optimal outcomes
Develop and collaborate on ongoing care/service plans with
providers and community resources
Monitor member status and update care plans as necessary to meet
goals and address unmet needs
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in care management
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
Must reside in/around Hillsborough, Polk, Hardee, Highlands, or
Manatee County, FL
