Job Description:
A company is looking for an In-Home Services Case Manager to
join their Pediatric Behavior Health Team in a remote capacity.
Key Responsibilities
Manage a caseload of clients aged 4 to 18, providing clinical case
management in home and community settings
Develop and implement treatment and crisis plans, and teach
behavioral interventions to families
Connect families to community resources and provide after-hours
crisis support as needed
Required Qualifications
Bachelor's degree in Social Work, Counseling, or a related
field
At least 1 year of applicable experience in case management
Clean driving record and applicable vehicle insurance
Ability to work independently and flexibly based on family
needs
Familiarity with evidence-based programming, such as Attachment,
Self Regulation, and Competency (ARC)
